1. Decision Log Entry

“Record a decision. Include: the decision, the alternatives considered, the evidence that changed my mind, what would reverse this decision, and the date. Verbatim – do not summarize.”

2. No-Solution Declaration

“State clearly whether a solution exists for this problem. If none exists, say ‘NO SOLUTION’ and explain why in one sentence. Do not propose partial workarounds unless asked.”

3. Blast-Radius Declaration

“Before acting, declare: the maximum write scope, the maximum spend, the undo path, and who to escalate to if the boundary is hit. If any field is unknown, say UNKNOWN and stop.”

4. Assumption Audit

“List every assumption this plan makes. For each: what evidence supports it, what would falsify it, and how confident I am (interval, not single score).”
